Life Management Support Assistance

Living with a life limiting illness can impact everyone in a family.  Little things can become big things making every day seem like a struggle.  Hospice of the Carolina Foothills can reduce the stress the hospice and palliative care patients we serve through the Life Management Support Assistance Program.  Our trained professional staff and volunteers are here to make life easier, and can provide the following:

  • Helping with daily household functions like paying bills, changing batteries, making appointments, keeping daily calendars, and making grocery lists.
  • Completing advance directives or the Five Wishes® with patients and their caregivers.
  • Identifying outside resources and making referrals for meal/food/nutritional supplement assistance-including delivery of food boxes from community pantries or senior centers.
  • Arranging for transportation to appointments.
  • Assisting with Medicaid application and eligibility, disability applications, and with the social security administration regarding benefits.
  • Determine resources to cover prescription medication costs that aren’t covered by the Hospice Medicare Benefit.
  • Securing life alert-assistance by reviewing available plans, making referrals to the appropriate service, and finding alternative methods including monitors and cameras for patients where appropriate.
  • Conducting a safety assessment to determine if furniture placement, rugs, etc. reduces patient safety and make recommendations to rectify the problems. Find resources for mobility aids such as grab bars, handheld showers, non-skid surfaces, and ramps.
  • Find resources for home winterization/weatherization for minor needs such as plastic over windows etc.
  • Communicate with the VA for patients in the system. Start the process for those eligible and not in the system.
  • Verify the current insurance payer for all non-Medicare patients.
  • Assist with funeral arrangements including working with insurance companies and funeral homes to identify less costly alternatives when needed.
  • Work with long term care insurance, Medicaid, and alternative sources of assistance regarding coverage and initiation of in-home care.
